Reporting an Incident & Ambulance Reimbursement Process

Incident Reporting

In the event of an incident/injury, club staff must complete the below Incident Report Form within 48 hours of the incident:

Ambulance Reimbursement Process

An additional service that AGF provides for its registered members is reimbursement to the club/athlete when an accident occurs during an AGF Sanctioned Activity, and an ambulance is required to be called.

NOTE: Any time an ambulance is called to a gym due to an injury, AGF must be immediately informed by contacting [email protected].

The following steps will outline how to submit an Ambulance Reimbursement with AGF:

  • Following an incident where an ambulance was required to be called, complete the Alberta Gymnastics Injury and Accident Report Form
  • Maintain contact with the injured individual’s parent/guardian and record notes as required
  • Following payment for the ambulance (whether it was by the club or the parent/guardian), scan and submit a copy of the invoice to  [email protected]

After confirming that the injured party is a member of AGF, a cheque will be mailed out to the payee to reimburse the expense of the ambulance.

Sports Accident Claims

Participant accident is NOT primary medical insurance. To make a claim, you must exhaust provincial health care and any extended health benefits before you submit. Participant accident insurance is an insurance policy provided as a benefit from AGF. It is NOT intended to replace any extended benefits plan. All participant accident claims will be processed and recorded as an insurance claim.

AGF Members looking to be reimbursed for medical expenses not covered by their primary insurance provider must take the following actions:

  1. A club representative must complete the Alberta Gymnastics Injury and Accident Report Form. This must be submitted within 48 hours of the incident. It is best to submit it immediately to ensure proper documentation.
  2. The injured individual or guardian should refer to the Participant Accident Claims Form for a full list of instructions and deadlines.  Please note that the first page is to be completed by the Injured Party and the second page is to be completed by the Attending Physician. The Alberta Gymnastics Injury and Accident Report Form can be attached to the first page rather than having to duplicate the injury information and circumstances.
  3. Submit all receipts and completed forms to AGF at [email protected]

General Liability and Errors and Omissions Claims

AGF member clubs who have been served with a lawsuit must contact AGF’s insurance provider immediately to begin the claim process:
