If you will be submitting a video of yourself as part of your coach evaluation, please ensure that any participants who appear in your video have signed the following form:

This form does not need to be submitted to AGF. Please keep on file at your facility.
THEORY + INTRO + SPORT SPECIFIC + EVALUATION = CERTIFIED
Following the completion of the three Gymnastics Foundations (GF) courses (including the online MED evaluation, and the NCCP Emergency Action Plan), you must assemble and complete your “Coaching Portfolio” which can be found in the GF Evaluation Package to complete the GF Evaluation application.
Step 1: You have attended the GF Theory, Introduction and Sport Specific course(s). You are also 16 years of age or older.
Step 2: You have reviewed and completed the Gymnastics Foundations Evaluation Package
Step 3: Fill out the application, upload evaluation requirements and make the required payment of $50. Once the application has been submitted you will be emailed a link to create an account to communicate with the evaluator.
Step 4: Wait to hear from your Coach Evaluator for pre-brief.
Step 5: Upload a lesson observation or request onsite observation. You will be emailed a link once your portfolio has been approved.
Step 6: Coach Evaluator de-briefs with coach.
Step 7: Upon successful completion, results of evaluation and certificate are sent to coach.
Please note that you will have up to 90 days to complete your evaluation. If you do not finish within 90 days from submission, you will be required to re-submit your evaluation package and fee again.
C1 TRAINING COURSES (Theory & Sport Specific) + EVALUATION = CERTIFIED
Following the C1 training courses (including the online MED evaluation, and NCCP Sport Nutrition Module), a coach must assemble their Coaching Portfolio which can be found in the C1 Evaluation Guidelines for Success
Step 1: You have completed the C1 Theory and Sport Specific course(s). You are also 16 years of age or older

Step 3: Visit application to upload evaluation requirements. Payment of $110 will be required at the time of application. Once the application has been submitted you will be emailed a link to create an account to communicate with the evaluator.
Step 5: Coach will upload a lesson observation or request onsite observation. You will be emailed a link once your portfolio has been approved.
C2 TRAINING COURSES (Theory & Sport Specific) + EVALUATION = CERTIFIED
Following the C2 training course(s), a coach must assemble their Coaching Portfolio which can be found in the C2 Evaluation Guidelines for Success.
Step 1: You are C1 WAG or MAG CERTIFIED.
Step 2: You have attended the C2 Theory and Sport Specific course(s).
