Travel Authorization

23 January 2025

AGF would like to remind all clubs planning to travel outside of Canada that a Travel Authorization Form must be completed. This form is required for all activities, including but not limited to competitions, training camps, clinics, etc.

Submitting this form ensures effective risk management by allowing AGF to:

  • Be aware of any travel taking place to assist with any issues that may arise during or after the activity.
  • Identify participating coaches or athletes and collaborate with you to highlight them on our social media platforms.

Submission Guidelines

  • Deadline: The completed form must be submitted at least 60 days prior to the first day of the event/activity.
  • Review Process: Once your form is submitted, you will receive a copy. The AGF office will review it to confirm all athletes and coaches are registered members in good standing. A confirmation will be provided within 10 days.

Insurance Coverage Notice

Please note:

  • If AGF is not informed of travel to an event/activity, no insurance coverage will be provided.
  • AGF’s accident insurance policy:
    • Covers incidents during approved events/activities on the field of play.
    • Does not provide travel medical insurance or cover incidents occurring off the field of play (e.g., social events, travel to/from the venue, non-competition activities).
  • We strongly recommend obtaining additional insurance coverage for areas not covered by AGF.

In the event of an injury, please ensure the AGF Injury and Accident Report is completed immediately.

If you have any questions regarding the Travel Authorization Form, please contact your program manager or the AGF office.
